What is Breast Augmentation?

Breast Augmentation is the most sought-after cosmetic surgery to enlarge the breasts and restore their shape usually by breast implants or sometimes by autologous fat transfer. It is an outpatient procedure performed by certified cosmetic surgeons. This surgery is also known by other names such as ‘augmentation mammoplasty’ and ‘boob job’.

Why is it done?

Breast augmentation may be the right solution for a woman whose breasts are too small, uneven, or not proportionate with the remaining body. It is the best option for those who have the following concerns:

  • A belief that their breasts are too small for their body
  • A feeling of self-consciousness wearing a form-fitting, swimsuit, or low-cut top
  • Clothes fitting to their hips but loose at their bust line
  • Breasts not filling out their bras
  • Breasts that have become less firm or smaller after giving birth to their child
  • Smaller breasts because of their significant weight loss
  • One of the Breasts is noticeably smaller than another
  • Sagging Breasts (in such a case both Breast Augmentation and Breast lift can provide the best results).

Who are good candidates for breast augmentation surgery?

Breast augmentation surgery is entirely an elective personal operation, and it is important that the candidate does it for herself and not for someone else, even if anyone has offered some profit for undergoing the surgery. A woman can be a suitable candidate for breast augmentation surgery if:

  • They are in good overall health and are not pregnant or breastfeeding
  • They have realistic expectations from the surgery and a positive outlook
  • They have fully-developed breast
  • Their breasts are asymmetrical
  • They are bothered by their smaller breasts
  • They wish to get rounder, larger, and fuller breasts after pregnancy, weight loss, or aging
  • They are unhappy with their upper breasts part appearing “empty”
  • One of their breasts has an elongated shape or has failed to normally develop.

The Procedure

Breast augmentation surgery is carried out under general anesthesia, intravenous sedation, or even regional anesthesia in a hospital or an outpatient surgery clinic. As soon as the anesthesia takes effect, the surgeon starts performing the breast augmentation surgery by either of the following techniques:

  1.     Breast augmentation using breast implants

Firstly, the surgeon makes incisions in inconspicuous areas of the chest to minimize noticeable scarring. The incision options include the crease under the breast (inframammary fold), along the areolar edge (peri-areolar region), and the armpit (axillary area). Depending upon the type of breast implant, the particular breast anatomy of the patient, the desired degree of breast enhancement, and patient-surgeon preference, one of the incision options is chosen.

Once the incisions are made, the next step is to place the chosen breast implant into a pocket created in the chest area either submuscular (under the pectoral muscle) or sub-glandular/submammary (over the pectoral muscle just behind the breast tissue). The breast implant could either be a silicone-based shell filled with silicone gel (called silicone implants) or filled with sterile salt water (called saline implants). The method used to insert and position the breast implants depends upon the type of implant, the patient’s body type, the degree of the desired enlargement, and the surgeon’s recommendations. In the case of a saline implant, first, an empty shell is placed in the breast, maybe through a cut near the belly button, with the help of an endoscope and when it’s in place, it's filled with salt water.

The third step involves the closing of the incisions. Breast tissues are closed with layered sutures and the wounded skin is closed with sutures, surgical tape, or skin adhesive. The results of this surgery are immediately visible. However, breast implants do not last for a lifetime and need to be replaced after several years.

  1.     Breast augmentation using autologous fat transfer

During this procedure, the surgeon performs liposuction to remove extra stubborn fat from different body areas having abundant fat cells such as the abdomen, hips, and thighs. The harvested fat from the patient’s own body is then processed, fat injections are prepared, and finally, the breasts are administered with the fat cells. This technique won’t work if the candidate does not have sufficient fat. It has also been noticed that not all transferred fat would survive the grafting process and sometimes the surgeon may need to again perform the procedure to retrieve more volume of fat for breast augmentation. This works only if mild to moderate breast enhancement is required.

What type of breast implants are there?

There are multiple choices for breast implants. The patient plastic surgeon will help one choose an option based on the size and form of the breasts, the patient's medical history, and the outcome one seeks from the procedure.

One has the option between saline and silicone implants. Choosing an implant type is often a matter of personal taste and surgeon opinion. One will also have to select the size and shape.

Breast Implants Containing Saline

Sterile salt water is used to fill saline breast implants. They may be prefilled at a fixed size or filled after surgery to allow for modest size adjustments.

Structured Breast Implants Filled with Saline

These implants are filled with sterile saltwater but have a structure, so they act as if filled with a soft, elastic silicone gel. These implants maintain their shape more effectively than conventional saline implants. These implants were approved by the FDA and Health Canada in 2014 and have been available since 2015.

Gel-Filled Silicone Breast Implants

These implants are available in various forms and degrees of firmness and are filled with a soft, elastic gel. All breast implants made of silicone gel are prefilled. If one is receiving a more prominent implant placement, the incision will likely be longer.

Cohesive Gel Silicone Gel-Filled Breast Implants

Cohesive gel silicone breast implants are filled with a cohesive gel composed of cross-linked silicone molecules, making them somewhat thicker and stiffer than conventional silicone gel implants. Additionally, they retain their shape better than conventional silicone gel implants. Also, in the year 2012, the FDA approved the use of these implants in the United States. Since 1992, these implants have become available in many parts of the world.

Recovery and aftercare

After breast augmentation, the patient would typically need one to two weeks to fully recover from the surgery. Initially, for a few days, the patient would have to keep their operated breasts covered with sterile bandages, wear surgical bras (the compression garment), and may also have some surgical drains. After the anesthesia wears off, the patient is likely to feel post-operative pain and discomfort, which can be relieved by painkillers. The breasts may feel tender or tight and sensitive to touch and the breast skin may feel itchy and warm. The patient may expect to face difficulty while raising her arm. Redness and swelling are commonly seen as side effects of the surgery but these disappear soon.

Some surgeon-recommended aftercare instructions:

  • Sleep and rest on the back in the inclined position to reduce swelling in the initial days after the surgery.
  • Avoid bending, straining, and lifting activities until the breasts completely heal. Walking is a must for a few minutes every few hours post-operation.
  • Taking shower is permissible between 1-7 days after the surgery.
  • Do not indulge in strenuous exercises and sexual activity for a minimum of 2 weeks.
  • Continue to schedule routine mammograms and perform breast self-examinations.
  • Resume work within a few days or weeks as per the nature of the job.
